Pragmatic Free Trial Meta

Pragmatic Free Trail Meta is an open data platform that allows research into pragmatic trials. It collects and shares cleaned trial data and ratings using PRECIS-2 which allows for multiple and varied meta-epidemiological studies to examine the effects of treatment across trials that have different levels of pragmatism and other design features.

Background

Pragmatic studies provide real-world evidence that can be used to make clinical decisions. The term "pragmatic" however, is a word that is often used in contradiction and its definition and measurement need further clarification. The purpose of pragmatic trials is to inform policy and clinical practice decisions, not to confirm an hypothesis that is based on a clinical or physiological basis. A pragmatic trial should aim to be as close as is possible to real-world clinical practices that include recruiting participants, setting up, delivery and implementation of interventions, determining and analysis results, as well as primary analyses. This is a significant difference from explanatory trials (as described by Schwartz and Lellouch1) which are designed to provide more thorough proof of an idea.

Truly pragmatic trials should not blind participants or the clinicians. This can result in bias in the estimations of the effects of treatment. This method could help overcome limitations of observational studies that are prone to limitations of relying on volunteers, and the limited accessibility and coding flexibility in national registry systems.

Pragmatic trials also have advantages, including the ability to use existing data sources and a greater chance of detecting significant differences from traditional trials. However, these tests could be prone to limitations that undermine their effectiveness and generalizability. The participation rates in certain trials could be lower than expected due to the health-promoting effect, financial incentives or competition from other research studies. Many pragmatic trials are also restricted by the need to recruit participants in a timely manner. In addition, some pragmatic trials do not have controls to ensure that the observed differences are not due to biases in trial conduct.

The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-labeled themselves as pragmatic and were published until 2022. They assessed pragmatism by using the PRECIS-2 tool, which includes the domains eligibility criteria and recruitment criteria, as well as flexibility in adherence to intervention and follow-up. They discovered that 14 of these trials scored pragmatic or highly sensible (i.e. scores of 5 or higher) in any one or more of these domains, and that the majority of them were single-center.

Trials that have a high pragmatism score tend to have broader eligibility criteria than traditional RCTs, which include very specific criteria that are not likely to be used in the clinical setting, and include populations from a wide range of hospitals. The authors suggest that these traits can make the pragmatic trials more relevant and applicable to everyday practice, but they don't necessarily mean that a pragmatic trial is completely free of bias. In addition, the pragmatism that is present in trials is not a fixed attribute and a pragmatic trial that does not possess all the characteristics of an explanatory trial may yield valid and useful results.
